Forms from the tax office, the social security administration and your health insurer all use this label for freelancers.
The fuller explanation
OSVČ stands for osoba samostatně výdělečně činná. If you work on a trade licence you are an OSVČ, which brings three separate registrations: tax, social insurance and health insurance. Being an OSVČ is a status, not a document.
